Your dental health is a priority and Downtown Family Dental of Baraboo is proud to offer a Dental Membership Program to patients that do not currently have coverage under a dental insurance plan. Our annual Dental Membership Program provides patients the option of a complete preventative dental care program.

Child

For children 12 & under

  • Routine cleanings (2 per year)
  • Routine oral exams (2 per year)
  • Bitewing x-rays (or routine x-rays)
  • Fluoride Treatment (2 per year)
  • One emergency exam per year (if needed)
  • 15% off all additional regularly priced services when paid by cash or check on the day of service
  • 13% off all additional regularly priced services when paid by credit or debit card on the day of service
  • 5% off all additional regularly priced services when paid by Care Credit*

Adult

For patients with regular dental cleanings (no periodontal disease)

  • Routine cleanings (2 per year)
  • Routine oral exams (2 per year)
  • Bitewing x-rays (or routine x-rays)
  • One emergency exam per year (if needed)
  • 15% off all additional regularly priced services when paid by cash or check on the day of service
  • 13% off all additional regularly priced services when paid by credit or debit card on the day of service
  • 5% off all additional regularly priced services when paid by Care Credit*

Periodontal

For patients with periodontal (gum) disease

  • Periodontal maintenance cleanings (3-4 per year as determined by your dentist)
  • Routine oral exams by your general dentist (2 per year)
  • Bitewing x-rays (or routine x-rays)
  • One emergency exam per year (if needed)
  • 15% off all additional regularly priced services when paid by cash or check on the day of service
  • 13% off all additional regularly priced services when paid by credit or debit card on the day of service
  • 5% off all additional regularly priced services when paid by Care Credit*

Important information about our Dental Membership Program

  • The dental membership program is NOT insurance.
  • We can only offer membership to patients that are not currently covered under a dental insurance plan.
  • Membership is for one (1) year, beginning the day of enrollment.
  • Enrollment can start any date you choose.
  • Cash, check, credit card, debit card and Care Credit payments are accepted.
  • Membership dues are payable in full upon enrollment and are Non-refundable.
  • Payments for additional dental services are due in full on the day of service. Members receive a 15% discount when paying with cash or check, a 13% discount when paying with credit or debit card and a 5% discount when paying with Care Credit.
  • Enjoy NO deductibles, NO pre-authorizations, NO yearly maximums and NO waiting periods!
  • Benefits are self-managed – it is the member’s responsibility to schedule all appointments offered in this program.

Is this an Insurance Plan?
No. Downtown Family Dental of Baraboo is not an insurance company. This is a dental membership plan for those patients without dental insurance.

Can I sign up my entire family?
Yes. Each patient is individual and can join anytime. You can have different renewal dates for every member of your family if you wish.

What happens if I obtain Dental Insurance while I’m a member of the membership?
Our membership program is only for uninsured. If you become insured, your membership will be terminated and dues are non-refundable.

Can my FLEX plan be used for the annual fee for reimbursement?
Most FLEX and Health Savings plans do reimburse for dental procedures. However, it is up to you to contact the administrator of your FLEX or Health Savings plan to confirm the qualified plan expenses and services.

I have Dental Insurance, can I join your program?
No. You are not eligible for this membership if you are covered under a dental insurance program of any kind.
