Insurance Estimations, Billing & Financial Options

Dental treatment is an excellent investment in an individual’s overall medical & psychological well-being. Downtown Family Dental of Baraboo keeps financial considerations in mind to prevent the costs of dental care from becoming an obstacle in obtaining this important health service. Part of our effort includes communicating our office’s policies related to estimating insurance coverage, processing insurance claims and collecting payment for the services provided. As your clear understanding of these policies is important to the patient/provider relationship, we ask that you review this document carefully and acknowledge your acceptance by signing below.

Insurance Estimations & Billing

We have a wonderful staff that will assist in informing you about the care you need, the costs related to the services proposed and the estimated portions that your dental insurance will help to pay. We file your insurance claims as a courtesy for being our patient and it is our pleasure to assist you with this process. While our office will file insurance claims on your behalf, it is your responsibility to understand your dental insurance coverage. We make every effort to verify benefits and obtain accurate estimates prior to treatment being scheduled and/or completed; but we have no way of knowing what exclusions exist on every individual dental insurance plan. All insurance companies state that verification of coverage is not a guarantee of payment and that all terms quoted are subject to change without notice. If your insurance carrier does not cover a rendered service or denies payment on a dental claim, you are responsible for any outstanding balance. As such, we strongly recommend that our patients take the time to become familiar with their specific insurance plan(s) so they have an understanding of what dental services will and will not be paid by the insurance company. In addition, employers often change/review dental insurance benefit plans annually. To ensure that our records are accurate and your claim is filed with as little inconvenience as possible, please notify our office of any changes to your coverage prior to any dental appointment.

Downtown Family Dental of Baraboo does not get involved in disputes between divorced parents regarding financial responsibility for their child’s dental expenses. When accompanying your minor child to our office, you agree to be financially responsible for the care we provide to your child, regardless of whether a divorce decree or other court appointed arrangement places that obligation on your former spouse.

Financial Options

All co-payments, co-insurance, deductibles and other patient portions are due at the time of service. A finance charge of 1.5% monthly will be added to all account balances over 60 days from the day of service:

• Cash or Check – We offer a 5% discount on cash or check payments received in full at the time of appointment. There is a $25.00 fee for all non-sufficient fund/returned checks
• VISA, MasterCard & Discover – We accept major credit cards for payment at the time of service or final billing. Automatic payments can be billed directly to your credit or debit card. Please speak with a staff member for more details
• Care Credit – For our patient’s convenience we are pleased to offer a variety of payment plan options through Care Credit. As they are subject to change, please contact our office for more information on the current ‘deferred interest’ and ‘extended low interest’ payment plans offered through Care Credit.
